When you complete our subsidy review request form, we collect the information you provide, which may include:

• Full legal name, phone number, and email address
• ZIP code, household size, age range, and household income range
• Your general situation (e.g., self-employed, early retiree, recently lost coverage)
• Your electronic signature and the timestamp of your consent to be contacted
• Your preferred time and, where applicable, communication method for follow-up

We do not ask for or store your Social Security number, full date of birth, or bank account information through this website. Any such information needed to complete an actual Marketplace application is collected securely and directly as part of the enrollment process, not through this site.

2. How We Use Your Information

• To contact you about the free Marketplace subsidy review you requested
• To prepare an estimate of your potential eligibility for premium tax credits and cost-sharing reductions
• To schedule and conduct a consultation call, by phone, text, or email
• To maintain a record of your consent to be contacted, as required by law

3. How We Share Your Information

We share information only as necessary to provide the service you requested:

• Service providers: We use a customer relationship management platform (Levitate) and an automation tool (Zapier) to receive, organize, and follow up on your request. These providers process your information on our behalf and are not permitted to use it for their own purposes.
• Insurance carriers: If you choose to move forward with an application, the information needed to complete that application is shared with the relevant insurance carrier(s) and, where required, with the Health Insurance Marketplace (HealthCare.gov) — this is a standard part of any ACA enrollment, whether completed with an agent or on your own.
• Legal requirements: We may disclose information if required to do so by law, subpoena, or other legal process, or to protect the rights, property, or safety of Mayfield & Associates, our clients, or others.

We do not share your information with data brokers, marketing lists, or unrelated third parties.

4. Communications & Your Consent

By submitting our form and checking the consent box, you agree that Mayfield & Associates and Brandon & Jade Sowder may contact you at the phone number and email address you provide — by call, text message, or email, including through automated or autodialed technology — regarding ACA Marketplace health insurance options. Consent to be contacted is not a condition of purchasing any insurance product, and message and data rates may apply.

You may withdraw this consent at any time by replying STOP to any text message, asking us to stop calling during any conversation, or contacting us directly using the information in Section 9 below.

5. Cookies & Analytics

This site may use standard analytics and advertising tools (such as Google Analytics or Meta Pixel) to understand site traffic and measure the effectiveness of our advertising. These tools may place cookies or similar technology in your browser. They do not give us access to your personal contact information unless you separately submit it through our form. You can control cookies through your browser settings.

6. Data Retention

We retain the information you submit for as long as reasonably necessary to provide the services you requested, maintain accurate business and compliance records, and comply with applicable insurance recordkeeping and consent-documentation requirements. If you ask us to delete your information and no legal or regulatory retention obligation applies, we will do so.

9. Children's Privacy

This website is intended for adults seeking health insurance guidance and is not directed to individuals under 18. We do not knowingly collect personal information from children.

We are not affiliated with, endorsed by, or acting on behalf of the federal government, the U.S. Department of Health and Human Services, the Centers for Medicare & Medicaid Services, HealthCare.gov, or any state-based health insurance exchange. 11. Changes to This Policy

We may update this policy from time to time. The "Effective date" above reflects the most recent revision. Continued use of this site after changes are posted constitutes acceptance of the updated policy.
