Mayfield & Associates is a licensed independent insurance agency. Brandon Sowder (NPN #20622977) and Jade Sowder (NPN #20595181) are licensed independent insurance agents in Florida, Alabama, Arizona, Michigan, Maine, North Carolina, South Carolina, Indiana, Ohio, and Montana. We are not the Health Insurance Marketplace, and we are not affiliated with, endorsed by, or acting on behalf of the federal government, HealthCare.gov, or any state-based exchange.

2. Eligibility and Acceptable Use

• You must be at least 18 years old to use this Site or request services.
• You agree to use the Site only for lawful purposes and in a way that does not impair or damage the Site or interfere with others' use of it.
• We may modify, suspend, or discontinue any part of the Site at any time, with or without notice.

3. What This Site Offers

This website allows you to request a free, no-obligation review of your potential eligibility for Affordable Care Act (ACA) Marketplace health insurance plans, premium tax credits, and cost-sharing reductions. Submitting the form does not enroll you in any insurance plan and does not create a client relationship until you and one of our licensed agents have spoken and you have chosen to move forward.

4. No Guarantee of Eligibility, Pricing, or Enrollment

Any figures, ranges, or examples discussed on this site or by our agents are estimates for planning purposes only. Actual eligibility for subsidies, actual premium amounts, and actual plan availability are determined by the Health Insurance Marketplace and the relevant insurance carrier(s) based on your verified information, and can differ from any estimate. You are always free to enroll directly and at no additional cost through HealthCare.gov or by calling the Marketplace Call Center at 1-800-318-2596 (TTY: 1-855-889-4325).

5. Accuracy of Information You Provide

The guidance and estimates we provide depend on the accuracy of the information you give us — including income, household size, and household composition. You are responsible for providing accurate and complete information. We are not responsible for consequences arising from inaccurate or incomplete information you provide, including any repayment of premium tax credits owed at tax time as a result of a change in your actual income.

By submitting our form, you consent to be contacted by call, text, or email as described in our Privacy Policy. You may withdraw this consent at any time.

These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-law principles, except where applicable state insurance regulation in your state of residence requires otherwise.
